As parents of young athletes, we understand how difficult and time consuming it can be to find a convenient and affordable location to have the physical performed.  That is why we are now offering Pre-participation physical exams for the student athlete at our offices. 

Please have the parent/guardian consent form signed prior to the visit.

The sports physical is not covered by insurance.
